Using a book as a model release with metadata

I'm trying to do something unusual with an Aperture book -- I want it to print out a model release with photo for me.

I've designed a book that will show one or more photos of the model and I've inserted the text of a model release. As extra organization and insurance, I want the model's name, phone #, email and date of photo to be inserted as metadata. They would be typed in the project but I want them to appear in the book.

I know how to insert a metadata field on the book page, but I can't figure out how to link the metadata. I follow the manual instructions precisely but no "pop up" menu allows me to link the data.

Also -- how can I save my book design so it automatically opens up when I select "New book" in the Aperture toolbar?

Next question: I want to save this design as a one page book and I'm having trouble getting rid of all the excess pages.
