iPhone 4S camera lens has a scratch

My iPhone 4S has a scratch on its lens, and the scratches show in all of my pictures.

I have the AppleCare+ plan.

What should i do?

Will the store replace my phone, or will it just replace the back panel? and how much will it cost?

I have many scratches on my screen and the back panel. and i would be really happy if apple replaced my phone. Will they? or what could i do so they replace my phone?

The Genius could decide to Replace the Back for free. Or decide to Replace the entire iPhone for $50. Or the cost of a new back without using the AppleCare + would be $29.
