Keyboard Shortcut Bar disappeared
The Shortcut Bar (also called Edit, Suggestion, Predictive or Quick Type Bar) sometimes closes or completely disappears and can't be opened or brought back.
This issue first appeared after updating to iOS 9. It takes two different forms.
1. The Shortcut Bar closes, but can't be opened by sliding up the Grab Handle:

The following workaround resolves the issue for most users.
Open Settings > General > Keyboards > toggle OFF then ON the highlighted options (i.e., Shortcuts and Predictive).
Note: Exact Keyboard layout and appearance varies from app to app and not all apps and fields support Shortcut Bar.

Also predictive/shortcuts can be turned off from the keyboard. Tap the globe then predictive, shortcuts, or both.

Doing it from the keyboard is app-specific.
