When you build a book in iPhoto from scanned in images be sure to use this tip [which i got from an Apple iPhoto person - thanks]. In AppleCare Knowledge Base Document 61409 they mention the issue with non 4:3 ratio images in the Picture and Classic-style books. To avoid these anomalies you can preview the actual PDF file that is sent to the book printer. Build your book as usual. In iPhoto select the Share option and then click on the "Order Book" icon. An "Assembling Book…" progress dialog should appear. When it has finished make the Finder active. From the Finder, select the "Go to Folder…" option from the "Go" menu. Type "/tmp" - in the tmp folder you should have an "iPhoto" folder. In this is a file with a name like "Book-36362400.pdf" – this is the PDF of the book. Open this up in Preview or Acrobat Reader and make sure all appears correct. If so, you can continue with the ordering. If your images appear mis-cropped or oddly aligned, it's time to go back to re-cropping in iPhoto or Photoshop. Anyway this is great tip to be able to see what exactly is going to be printed.
