I think I understand your question and problem, but...
There is a preference for country in the Address Book preferences, but I am not sure whether that will be helpful to you.
Perhaps it is because I am in the US, but I do not have separate field for middle name in the Address Book application.
Have you tried to put the middle name as part of the last name field (van der CDEF)?

I did not fix the issue. I did delete my user account, and created a new one, now the issue is gone. I think there was a broken file or whatever causing the problem. But it seems I am not the only one here. Deleting a user account is not an elegant way of solving a problem, but it worked for me within 10 minutes.

I have tried deleting "com.apple.mail.plist" but that does not solve the problem.
I don't want to delete my user account, don't know what I will loose with that...MacBook Pro 2.66 GHz Intel Core 2 Dua, Mac OS X (10.6.1), 4GB
