I am not sure I understood but what you describe seems the way it should be: you have the plus icon only on the last field of each "field family", i.e. phones, email, people, addresses if it's not empty.
You can change the label for each field in a family, I mean, a phone can be labeled as office, home, mobile, fax and whatever by clicking on the label and choosing as you need.
If you have at least an empty field in a family you don't have the plus icon, that appears as soon as you fill that field in.
As said before, possibly I didn't understood the issue: possibly clarify it more, so I try to reproduce it!

I remebered that -as default- both points (gree and red) were always visible even with empty fields.
No documentation about the problem, but -anyway- everything is ok, now.MacBook Pro 15" - 2,53 GHz Intel Core 2 Duo, Mac OS X (10.5.8)
