A clarification: The play button is double when the disc is viewed on my tv. When viewed on my computer using the DVD Player app, all is as expected. I notice that when loading, the aspect ratio visiblychanges from 4:3 to 19:9. I think aspect ratio has something to do with the double button... the red state of the button is not scaled correctly; in fact, it's stretched.
I have reinstalled iDVD and updated it, and repaired permissions, and verified the disc. Using a Mac Pro 2.66 quad-core Xeon. I am now going to try burning a disc with the same theme on another computer.

What theme are you using? Have you tried a different theme to see if it's just theme related.
Try the following:
1 - delete the IDVD preference file, com.apple.iDVD.plist, that resides in your

If that doesn't work post a screenshot of the button before and after so we can see exactly what you're referring to.

Since the project plays OK with DVD Player then the problem is either with the burn process or, as you mentioned, with the set top DVD player. See if you can find another set top DVD player and try it there. If it still offsets then the burn process is looking suspect.
In that case burn to disk with Disk Utility at the slowest speed available to assure the best burn quality. Always use top quality media: Verbatium, Maxell or Taiyo Yuden DVD-R are the most recommended in these forums.
