This worked for me when I could not get a document to upload to iCloud. My document showed the up arrow icon in the corner of the file when looking at the thumbnails of all the documents in the "start up" view in Pages - but it wouldn't upload. Maybe it will work for you.
Try making a little insignificant change again on the document on either the phone or the Mac. Or create a new test document - anything to make iCloud have to sync again - with something new - Don't just unsync and resync current files - make that sucker iCloud work!
See if that brings the other document back to life.

Anyone have any other ideas? I really need this document on my iPad and alas we can't access the iCloud via the iPad. Is this a case of booking a visit to the genius bar?

I paste below the solution to my own very similar problems - having just lost a day and most of the night thinking of possible solutions I hope I can save you some time . . . Good luck
I HAVE THE ANSWER!
Apologies for shouting but this is my solution:
1) Make sure the 'good' files are backed up first
2) As Winston said, turn off icloud in pages and icloud settings
3) As Winston said, reboot any devices
4) The solution . . . open Safari browser and sign in to icloud - the key here, is that I was previoulsy using Google Chrome as my browswer. I saw ALL my files there, included the ghost files that were not visible in Chrome. I deleted the lot, one by one. When I switched icloud back onto ipad and iphone all the ghosts were gone. iCloud is working on all three devices (PC included) and I have cancelled my Genius Bar appointment (hopefully not too early). I hope this saves others the day and night its taken me!!

Swing and a miss for me I'm afraid. I don't have any ghost files on iCloud. Moreover I don't have any problem Pages documents on my iPhone. Just the iPad 2. Think I'll take the iPad 2 to the Genius Bar

Sorry to hear it didn't work for you. Like you, I only had the ghost on my iPad 2, although latterly I did get one on the Iphone too. They all went once I'd deleted the files on iCloud that we're only visible using Safari.

Ok visit to the Genius Bar it was then. They confirmed this was not the first one they had seen. The solution is to wipe and reinstall the iPad. There you go

I've suddenly been unable to access any iCloud documents from my iPad just since I last used it 3 days ago. All is fine on Mac and on iPhone. but Pages on iPad cannot get anything from iCloud. It will not upload to iCloud either.
I tried going to App Store and downloading Pages again, but it seems we can't do that, either.
When you "wiped" the iPad, did you lose all data from other apps?
