I have this same problem but the box for Manually Manage Music and Videos is not checked since ages.
I just noticed only now that iTunes is not syncing the play count on my iPhone and it counts only playbacks on my Mac.13" MacBook Pro 2.4 GHz Unibody, Mac OS X (10.6.5), iPhone 4 (iOS 4.2.1)

i have been manually managing music since iPod Mini.. then iPod Classic 6th gen, iPhone 3G, iPhone 3GS.. and now iPhone 4S..
playcount gets synced back everytime until iPhone 4S when Apple breaks the feature that i really want... i only able to listen to one song now so i can increment it manually on the iPhone then sync...

Yup. Memiehic is correct. It is even worse.........When you sync your iphone to itunes (and it doesn't matter if it is a 4S, 3GS or 4---it is itunes and OS5), the plays on your iphone will revert back to whatever was on your computer. It also effects the last date played as well. This is a problem for me, since I have a self updating playlist on my iphone that only plays songs with zero plays. Once I sync with itunes, any song I have played since the last sync reverts back to zero!
APPLE PLEASE FIX THIS. Worked great before IOS5

It's not just that play count and date played don't sync back to the computer; they do not change on the ipod at all when music is played. I manage my music manually so I use another app to copy playlist data back to the computer (I've been doing this for many years), but now there is no playlist data to copy.

Playcounts work if it is on Auto Sync Music, and not Manually sync... this probably is my LAST iPhone if they break core functions that works since iPhone 3G, i DONT WANT TO AUTO SYNC MUSIC (interface is way too slow in iTunes, and not that accurate), i want to Manually Sync and still have playcount updated...
another rant: and i hate that in iTunes the "Get Info" button was moved to 2nd choice, i NEVER use the 1st choice of "Play"... i always double click (I know iTunes is foremost on Mac OS X, not sure if you double click there means edit the song title)

i realise why Apple has changed this behavior....
Now you can add music on multiple computers! (support page updated after iOS 5 introduction), thus playcount is no longer working when you are in Manually Manage Music mode... but instead of tracking individual libraries, they just kept it indepedent... and break the feature.
Apple should've explained this... i lost some good playcount on my songs
