Screenshots occur when you quickly press and release the home and on/off buttons at the same time.
It's unlcear what your are doing when you "unlock" the phone. Normally, pressing either the home or on/off buttons wakes the phone and you slide the unlock slider to unlock.

It has happened several times today and I've been paying attention to better describe it.
It happens when I pick up the iPhone and press the Home button to unlock it. The Home button is the only unlock method I use, although I am aware I could use the top button, too. But as soon I press the home button, I hear the shutter sound.
I think that's why some of the screenshots are totally black while others are of the unlock screen--sometimes the screen capture is so fast it gets it before the unlock screen is actually drawn.
And as I sai, this doesn't happen every time. It's maybe one in ten.

This happens to me a lot and it's very annoying as I have to delete the lock-screen pictures all the time.
I haven't yet found a way to reproduce it on-demand but after reading these posts I've been monitoring closely and am sure I'm not pressing the on/off button at all.
I now suspect it has something to do with the home button in combination with phone motion. I've noticed that if I don't hit the home button until I've got the phone in front of me that this rarely, if ever, happens. Most of the time this happens when I'm in a rush and press the home button while the phone is in motion from its holster to the viewing position, but I've not been able to replicate this either.

i got the iphone 4 and never had that problem,
now i have gotten iphone5 and i have the same problem, that my phone when press the homebutton to unlock it takes screenshots of the lockscreen.

I've got the same on my iPhone 4S ever since IOS 7 is on it.
Just taking it out of my pocket, pressing the top on / off button and a picture is taken.
After that, just a black picture is in the library.
Does it every day...
