How can I ensure this doesn't happen again?
It is an electronic device, you likely cannot.
Perhaps not being where you usually are had something to do with confusing the phone.'09 Mac mini, iPad 2, iPhone 4S, Mac OS X (10.7.3), iOS (5.0.1) Mac fanboi since 1984!

The automatic time change comes from the carrier/carrier's tower your iPhone is connected to. There could have been a problem with this function at the tower your iPhone was connected to in conjunction with the daylight savings time change.
The Time Zone shown under Set Automatically when Set Automatically is on has no bearing on the date and time being set automatically based on the tower your iPhone is connected with.
When I need to ensure I'm awakened, I have two alarms set. I've been using an iPhone for my wake up alarm beginning with the original iPhone nearly 5 years ago, but I also have my clock radio alarm set on my night stand set as well M-F.

Found some information from folks who had issues with the time change last fall. Here was the advice'
If your iPhone didn’t switch to the proper time for DST, you may have have one of the following issues:
- Disabled a Time Zone setting
- AirPlane mode may be enabled
Check to make sure automatic time adjustment is enabled (Settings > Location Services > System Services > Setting Time Zone ON), that Airplane mode is turned off, and then a simple reboot of your device is usually enough to fix the problem.'09 Mac mini, iPad 2, iPhone 4S, Mac OS X (10.7.3), iOS (5.0.1) Mac fanboi since 1984!

Thanks a ton for this bit of advise @Dah*veed.
I had a huge challenge while I was travelling! Mis matched times and calender reminders. Then after checking online, I saw a few posts that asked to trade in the 4S for a new one.
So I did.
And to my shock - no luck.
Then came Dah*veed!!
Thanks for the simple solution.
If the guys at the Apple Support store saw this, they would have saved themselves a trade in.
