Edward, I am running 10.6 on Windows and it is working correctly. As you know, the Comments field must be visible in order to be included in the search.
Try a system restart, and then see if it works. That may well clear the problem. If not, you have apparently discovered a bug.

It works in the main Library, but not in any Playlist type.
If I create a new Library it works as it should in a playlist.
There is clearly something wrong with the current library index but I don't know what information I'll lose if I start over.
Is there a way to repair or recreate just the index ?

Edward, If it works correctly on a new library, it is more likely a result of something getting stuck or corrupted rather than a bug.
Before you do the nuclear option of moving all your stuff to a new library, try to give iTunes a "kick" in a few different ways. Hide comments, change the search oval from All to Artist, run a few searches and then restart the machine and change it all back. Silly as it sounds, things like that have been know to work for various problems.
