I also have this problem, but it happens after I have plugged in my headphones.
From restart of phone, it works fine. Then I plug in my headphones. When I make a call, the speaker goes
on automatically. And if I turn of the speaker, I hear nothing. I then unplug the headphones and the problem remains. Speaker goes on automatically.
When I restart the phone, it´s all fine again.
I use the Apple in-ear plugs headphones.
I also have the "I hear myself" problem when making calls.
Software problem, or should I get a new phone?

Glad to hear I am not the only one. I'm gonna wait until 5.1 or 5.02 to come out to see if this problem persists, to confirm whether it's hardware problem or not.
Have you tried doing a wipe? I wonder if my previous settings from iTunes is causing this too.

You didn't say whether you have checked settings>general>accessibility>incoming calls and set it to default. iPhone 4S 64Gb iOS 5.0.1

I think it won't be something with your old itunes settings. Because I have the same problem but in a very clear case. New iphone, new laptop, new itunes.

I had the same problem, this is how i solved it............
Settings < General < Accessibility < Incoming Calls < Should be set at Default to correct the problem.
