Make sure you're not within an app, you can't enable mirroring there. Double tap home button, swipe (left to right motion) to bring up airplay, tap ATV and then toggle ON mirroring
Make sure Apple TV is up to date

The Airplay up arrow icon does not appear on either my iPad or iPhone. All other functions of Apple TV are normal.

My issue was expecting the Airplay arrow to be in plain sight. I finally found it by scrolling the bottom line of apps to the right. Why didn't Apple put this in the instructions? The other mystery I finally solved was on the initial setup screen. Discovering that in order to submit my username and password you had to scroll the bottom line to the right to get the Submit button to highlight. I'm sure Jobs never reviewed this setup or there would have been blood on the floor.

This was already covered if you had read this thread, and the link provided more detail on it. It is in the support literature. You can see the airplay icon 'in plain sight' whenever you're in an app that supports it. However, to enable mirroring you have to go through the multi-task bar.
