How, exactly, are you connecting it to your car stereo? Bluetooth? Aux jack? Dock connector? Have you tried turning up the volume on the phone?

With the dock connector. It's a pioneer head unit with the dock attachment. I have the iPhone 4 with ios 5.1. I plug it in and it will charge but when I go to play music it will play through the iPhone speaker.

Many of the docks in cars are not compatible with the iPhone. They were designed for the iPod. Check the documentation for your head unit and/or contact the manufacturer to see if there is an update available that will provide compatibility.
