Reset or restart the device.

How many times do I have to do that? Total restore as new phone didn't work. Genius told me to try that. Reset doesn't work. Any one have any ideas?

I had the same problem. Tried a hard reset, holding down the sleep/wake button and the home button untill the phone reboots. No good. Then tried going into Settings > General > Reset and tried every combination of reset. Then tried a restore from back up, then tried a total wipe and reinstall of the firmware through iTunes. None of this worked at all.
Eventually I bought a new camera unit with ribbon cable online and took the phone apart myself and replaced the camera unit. The camera now works perfectly, so after all that it was a case of a hardware component simply giving up the ghost.
This proceedure is not for the faint hearted and will invalidate your warrantly, so obviously if your phone is still in warranty just take it into an Apple store instead. If like me you have no warranty then google the help sites that tell you how to replace the camera. As a last resort for me this found the problem.
