Hello - same problem here on Snow Leopard. It used to work perfectly fine for a long time and I recently repaired the disk after a power outage and this happens now. The disk verifies OK and there are no other issues with the system. This is odd.

The problem only happens when the workflow is saved as a service as indicated by tmstacey. I created an application with the same workflow, drag and drop on it and all is renamed fine with 0001, 0002 etc. Only when used as a service is it skipping like 0001, 0003 etc.
Anyone else having the same problem? Anyone at Apple knowing anything about this?

I've got the same issue as above, only when used as a service... for me it started with Lion and was working fine with Snow Leopard. Anyone have any other thoughts on this?
