I was able to finally catch this happening on video. Had my iPhone propped up on a box and was just filming until it happened. I apologize for the audio, as I was watching Tekzilla on my other computer
Do you think this is an issue with the RMBP? Something graphics card related? I can't seem to figure out why this randomly happens.

That's quite odd. Does it act the same with other monitors / televisions? I've got a few HDMI - DVI and HDMI - Display port (the ugly large style), i'll test mine out and see if it's exhibiting a similar issue. Do you have an HDMI tv else where in the house you could try an HDMI - HDMI cable with? I'd imagine if it's graphics card related it'd reproduce the same issue.
I'll test mine out and post a response.

I've not heard nor read of any problems such as you describe (and document). I would try another adapter. I have the mini-display (Thunderbolt) to HDMI adapter from monoprice - http://www.monoprice.com/. (Adapters here.)
It works well with my late 2011 MBP and I assume that it would work with the MBP-R as well.

Four35ive, I have an HDMI TV I can try it on. The problem is that this happens so infrequnetly, that it's hard to reproduce. Let me know if you have any results on your end. Thanks!

clintonfrombirmingham, I'm not using any dongle type adatpers, just straight HDMI to DVI cables. I've tired a few of them over the last day and I have been able to reproduce this problem only 3 times, but it did happen at least once on both cables.

Well I do have two adapters on order from monoprice right now. But those are the the mini displayport to dvi. So that wouldn't really solve the issue if it was HDMI related.
A member on another forum suggested it could do with the fact that I was using it on a 22in monitor at 1680x1050.
This is what he said,
"Even though HDMI supports that resolution, it might be an issue since it's a 16:10 signal and not a 16:9 video signal.
1680x1050 is not a native video signal, it's a computer signal."
Not sure if that could be the reason. I have been running it on my 50 in TV right now with a standard HDMI input and haven't had any of the issues.

I do have encountered the exact same problem on a rMBP linked via HDMI to DVI (Amazon Basic) cable. The 22" Eizo (1650x1050) display goes black for a second, or even shows snow. I also testes at least two cables and was able to reproduce the default on random occassions.
I had the same guess as your source, that HDMI does not entirely support PC-monitors but couldn't confirm it. After all I still don't know if this is tolerable, or happening due to hardware-defects.
