Perhaps your Outlook 2007 software isn't ready for iCloud and needs an upgrade.

I'm not sure if that is the issue.
If I create an entry in my shared iCloud diary in Outlook it appears in that calendar (colour coded correctly) on my iPhone. So Outlook 2007 is connecting with iCloud and pushing entries to iCloud, but it doesn't apppear to be working the other way around.
Incidently I just created a new appointment on my iPhone in the shared calendar and that hasn't appeared in Outlook, so it doesn;t appear to be related to my wife's instance of the shared calendar.

It looks like the compatability is one way then... Things I do on my Exchange calendar are visible in iCal/iPhone, but things I do in my 'shared calendar' on my (on my wife's) iPhone/iCal are not visible in Exchange.
I was really hoping that iCloud had better compatability with Exchange.
It seems that when it comes to calendar integration iCloud has not really advanced from MobileMe's capabilities.
