Had the same issue (showed installed in AppStore, but was not actually updated). Apparently in my case, it had to do with the Apple ID that I was using.
I had used my AppleID to purchase the MtnLion and was trying to update to 10.8.2 on my wife's macbook pro. After struggling for about a day realized that the Macbook AppStore was defaulted to her Apple ID for the App Store. As soon as I signed her out and signed myself in, it continued with the installation.
Am updated to 10.8.2 now on the MacBook Pro and the Mini.

Likewise. I got a software update notification for Xcode and iPhoto on Sep 19. I accepted both but iPhoto was rejected because it is an update for 10.8.2. But 10.8.2 did not appear in the update list.
I just checked today (Sep 22) and the 10.8.2 update is there. I am dowloading it now.

The 10.8.2 update does not show for me either, and the direct download will not install. I am on a Mac Mini late 2012...
