Problem with the apple wired keyboard as opposed to some other USB slots, is that you can only hold the sides of the connector, not the top and bottom, so you have a lot less grip, its fine in my laptop and wall charger. The holes in the bottom of the USB connector really need to be filled in.

When I received my iPhone, I immediately plugged it in to charge, not thinking it would already have a full charge, which it did. Once I realized it was already charged, I went to unplug the USB cable from the wall wart and it was extremely tight. The white sleeve over the connection started to slide off, but I was able to get it out. I debated whether to try it in my laptop, but carefully plugged it in and saw that it wasn't such a tight fit and it worked fine. I was able to remove it, but didn't pull it straight out-more rocking sideways, just to be safe.
I went to the Apple store yesterday to exchange the cable. Their response was "Wow". Mainly because they had not seen one like that yet and it meant they would be sending to back to Apple and pointing out a problem to the engineering department. I did tell them that according to the forums, mine was definitely not the first and some were having worst problems than this.
Because Apple does stand behind its products, l will not be surprised to see a recall of the initial Lightning cables. My new one is still tight, but not as bad as the first one. When the other two phones arrive, it will be interesting to see what the cable is like that comes with them.

So went to apple store as usual they have heard nothing about this. Still really annoyed they still have no accessories. Picked up another cable on its own design is exactly the same.

LIGHTNING ADAPTER has fused to the USB port that is factory installed in my car. It will not release from the port and the port is no longer working as a result of my efforts to retrieve the USB cord. I have been an iPhone owner since the first release in 2007. I have never had a problem with my Apple products and have remained an enthusiastic, loyal customer. It is my understanding that Apple only has "extra" USB cords in Apple store. I live in a rural area without access to an Apple Store. Retailers and Carriers did not receive shipments of extra charging cords. This is unacceptable. I am absolutely at a loss because my port in my brand new car is no longer functioning and will likely have to be replaced, AT MY EXPENSE. My "Lightning" cord is literally stuck in my vehicle. My phone is currently sitting at Radioshack being charged so that I have at least a charge when I head out of town on business early tomorrow morning. Suggestions please as I am a VERY unhappy customer!

Mine is stuck in my car. I called Apple for assistance, and they told me to take my car into the dealership. I'll probably also try taking it to the Apple store to see if they can assist as well. If it kills my USB port like PP's.. that'll be REALLY upsetting.
I'm going to try cutting out a piece of plastic from a water bottle to wiggle it out. Thin cardboard didn't work for me, too flimsy.

I received a call from Applecare support and they are aware of the problem and looking to get some of the problematic cables back to see the problem.
I did indicate the problem is fairly apparent and is visible in the OP pictures that the bottom detents on the usb client plug are too deep and rather than allowing the fingers in the usb host port from retaining the plug, they are dropping all the way in and locking in the plug.
I used a 005 feeler gauge on my Acer to lift up the fingers in the host port and remove the usb clent plug.
While I wait for a replacement cable, I've placed a cutout from cellophane packing tape over the bottom of my usb plug to prevent the fingers in the usb host port from again locking in the usb plug.

I actually broke my usb extension that I normally use to charge my phone while in bed. I had to rip the usb sleeve out of the extension cord to get my lightning cord out.
I read where they would be providing replacement cords. That would be nice.
But I tried to go buy another cord at the apple store on saturday and there were none available.
Hopefully soon someobody will get them in stock.
I'm used to having like 10 of them.

The deep notches are the source of my problems too. I purchased another Lightning cable and it has the same "deep notch" problem as the cable included with the iPhone 5. See photos:
OLD "dock cable" versus NEW "Lightning cable":

happened to me here at work, called Apple and they gave me basic instructions on how to remove a USB cable (yea, I think I know how to do that!) she's stuck in there good...wiggled it, still stuck, firm pull still stuck! Gonna have to email back the Sr. Advisor that is taking care of my case and get a new cable hopefully since I don't wanna ruin this cable and be without one for the days it will take to ship me one. Worked fine at home in my iMac 27" but here at work in my relic PC no dice!

I exchanged my cable the day after I received my phone since the white sheath pulled away from the USB end when I tried to unplug it. I could still get the plug out, but not very easily. The new cable is still pretty tight, but I now just make sure I do not pull it straight out, and hope that Apple decides to do a redesign of the USB end.
One other thing I noticed on my cable. If you look at the USB end of this cable and one of the other cables straight on, you will notice that the space below the foam "spacer" is narrower on the Lightning cable. That was what I thought was causing the problem at first, until everyone mentioned the deep notches.
Did anyone else notice how then the white plastic sheath is compared to the one on the ipad cable or the other iphone ones? It almost feels like a urethane type material.
