Yes I've been experiencing a similar problem. Not sure about leaving the message app open, but it pretty reliable that if I clear all notifications, the next incoming text will be audible, but the 2nd, 3rd, etc will not sound an alert. If the phone is not "locked" or "asleep", then all text notifications are audible. This (only first text dings problem) only happens when the phone is locked.
Please keep posting if you have any new ideas to try or if for some unexpected reason, Apple is able to help.

You won't get alert sounds when the Message app is open and the phone is not locked. It's always been that way.
If audible text alerts aren't coming at other times, there is corrupted data/settings in the phone. What troubleshooting steps described in the User Guide have you tried so far?

Just to restate the steps to recreate the issue:
1. Clear all notifications
2. Lock phone
3. Have someone send a text, Alert is heard
4. Have same person send another text, Alert is not heard.
I don't recall if the Message app is "open/active" before the screen is locked.
The phone has been restored to the default settings and the problem still persists

In case I wasn't clear, the problem I'm describing happens when the phone is locked. I'm aware that you won't get a text message sound if the phone is not locked and the Message app is open, but I should always get the sound when the phone is locked. But more often than not, I don't.
For me, this behavior started with iPhone 5. Things were fine on my iPhone 4 and previous phones.
I restored the phone to original settings and the problem still exists.

Yes, this is still happening to me. When I receive a message using iMessage I sometimes never know if they sent it. I can go back and look and see that the message I was waiting for is there but I was never alerted. This does not happen if it's regular text messaging. I also notice it's usually if I receive a quick message from someone right after I sent a message then locked my phone. It's very annoying to have to keep unlocking my phone to see if I received a message because the iPhone is not notifying me that one has arrived.

I have this problem too and not just with iMessage. Mail, Reminders, and Phone all show the same behavior. I don't believe the issue is app specific, I think it is system wide. It has been an issue as long as I have had my 5c, which is less than a month; notification sounds have never come through reliably.
I don't receive a lot of notifications at once; I have had this problem with first notifications. I get the most notifications from Mail and have encountered this problem many times. I have had location based reminders not go off at all or go off some time after arrival. I have received calls without the phone ringing. I have double checked the settings and the ringer volume many times. I would estimate this problem happens more often than not with any alert sound, but I do sometimes get audible notifications as expected.
I have not restored the phone yet because it was brand new, but now I recall how when upgrading the OS on my 3g I immediately had to do a restore because of glitchy behavior, so I wonder if it is a good idea to alwasy start with a restore regardless. So I will do one and report back if this resolves the problem.
My little sister reported having this problem with an alarm set in Clock. Of course I can not confirm this was not user error or an excuse for being late to school

I have the same problem on my iPhone 4S, where my iMessage alerts are not occurring & when I open my phone no number is showing on the icon even though I have received several iMessages, this has only started since i down loading the new operating system, I have checked every setting etc ....very frustrating!
I went into the apple store and unfortunately they couldn't help either, they check all my setting all ok! but they couldn't give me a solution to my problem.
