And, I'd avoid the temptation to update everytime Apple offers one. With the Shuffle a good policy to follow is: "if it ain't broke, don't update it."

What you get will depend on your own wants and needs, but the two I find indispensable are the sports case/armband (already recommended) and a small speaker system. The one I got is at the very bottom of the quality spectrum, but for what I use it for (mostly playing podcasts in my shop/garage) it's perfect.
I guess I'll also second the other advice about not updating when you don't need to; I don't usually follow that advice, but I do at least let things sit there for a week or so after Apple releases them so I can see what kinds of problems folks might be having before using to update my own Shuffle.iMac G5 17" iSight, Mac OS X (10.4.4)
