Got the same issue. I thought it was my imagination, but every time I re-sync to iTunes, apps that I deleted months ago keep reappearing.
The only advice I can find is to turn off App Store Auto Downloads - I have this off for everything except Music, and still the Apps reinstall....
How hard is it to remove an App?

Have you deleted them from your iTUnes?
First, you decheck them in iTunes to remove them from your device. Then to delete the app you can delete it out of iTUnes. IT'll still show up in your purchase history but as a 'ok, you bought access to this and it's lurking out in the cloud if you ever want it again' type of app.

Nope, you should certainly not have to do that.
I have 2 devices syncing back to my iTunes. If I want an App on one and not the other, I can't delete it from my iTunes as it will be re-added as soon as I sync the other device, that still contains the App, back. Then resyncing the device that I don't want the App on will cause it to be pushed back again?!?
This used to work... Wonder what has changed. Is it iOS6 related?

Oh, and as a further FYI ... My iPhone (same iOS version, same iTunes) doesn't show this issue. Apps stay deleted. It's only the iPad that seems to keep reinstating them.

What I mean by deleting from iTUnes is, for example, with your iPad attached and chosen in iTUnes, navigate to the apps panel and then choose which app to take off. Then sync and it should be gone from that device. (I have several apps eligible for both my iPad and Touch, but some on one, others on the other, and it maintains it quite well.
NOw a few weeks ago I did notice that I deleted an app from my touch (can't recall if it was pre or post iOS6) and the app came back. I haven't been in the mood to continue to try and remove it to know for sure if it's going to stay gone if I delete it again.
I have had apps that I've tried out and deleted from all my devices. And I kept getting updates to those apps, until I 'hid' them in iTUnes I think and they do show up on my purchased page, but grayed out to show that they're not active.
There may very well be an iOS6 bug. I only have 6 on my Touch, not my iPad so can have different reactions on each device.

I'll take a look tonight, I am willing to bet that the app is already not ticked on the iTunes list.
Ooooh I wonder... Here's a thought... I wonder if iTunes is set to download App updates. Perhaps an App update arrives between sync sessions and the app is being deleted off the iPad and then sync'd with the upgraded app when reconnected. I'll check that out too and report back!

Grumble grumble... there's a bet I lose. So a deleted App can appear with a tick next to it in the iTunes app list?!? ok, this has to be a bug, never had to do that before.
And my iTunes isn't set for App update auto downloads. So this is gonna keep happening, yeah? I need to maintain a list of apps I'm removing (somewhere) and manually remember to uncheck these boxes in iTunes when I connect it? Hohum...
