You seem to be having a lot of issues with your iPhone 5. First step would be to Restore the iPhone with iTunes on computer. See if all the problems are resolved. If not, you have full Warranty, make Genius reservation and take iPhone to Apple for resolution.

I made a reservation at the Apple Store for this evening. We'll see what they can do to help me. I have AppleCare+, so I should be covered if they need to replace it.

I am having the same problems as well. Just started the other day, upgrading to 6.0.1 was no help. Guess I will make an appointment at the Genius Bar as well.

Went to the Genius Bar today, it was a no brainer, they said it was a hardware problem, gave me a new 16GB Black iPhone5. Nuff said.

They told me that it must be hardware related too. I'll be going back tomorrow to get mine replaced as well. I wonder why this is happening....I'm going to be so upset if the new one they give me has the same problem.
