No wonder I haven't gotten any replies on this one. Spent a good hour plus with two levels of Apple Care and they are escalating it up to the engineering dept as they are stumped so far.

I am having the same exact issue that you're describing and am frustrated.
I read in another discussion thread that in order to get rid of the duplicates you need to keep all of your contacts in either the "On My Mac" address book or the icloud address book. If it's in both, then you'll see the duplicates. The person recommended just using icloud and removing all the contacts from "On My Mac". They then said to change your default preference to use icloud, instead of "On My Mac".
In reality, I don't like that answer because I want to keep my contacts in both places.
As far as the down arrow, I haven't researched that one yet. I guess it's not bothering me enough yet.

I do not see the duplication in my previous recipients list, but I do notice that the autocomplete duplication only occurs for those emails that are listed in my previous recipients list. If the email address is not listed in my previous recipients list, it does not duplicate.
With that said, I suppose if I deleted it from the previous recipients list the autocomplete duplicates wouldn't occur, until it showed back into the list again...which I imagine would be the next time I emailed that address.
And to answer your second question, yes all of the duplicated email addresses are in my Address Book.
