Most note taking apps have "wrist protection" which means you can rest your hand on the screen. My favorite is Noteshelf. A stylus on a capacitive touch screen can't have a small end. Adonit's Jot is a pretty good work around. Many note taking apps also magnify the writing area so that even though you write large, it shows as a more normal size.
If you want to take notes on paper and have them digitized, consider Livescribe's Sky pen
I take most of my hand written notes on an iPad using Noteshelf. I find it much easier than paper. My iPad has a battery life of about 10 hours.
