If this service request is through Apple, they will send you a box with mailing label already filled out and ready to go. Do not send it without this box as the label contains tracking information, so that they know where and who to send it back to.

Contact Apple Support, start here: www.apple.com/support/contact
Once you have contacted them, they will send you a postage paid and addressed box to return to them. You should normally get the box the next business day after you set up the repair.
