Go to settings, general, accessibilty, learning, turn off guided access.

Hold home button and lock button for 10 seconds to reset iPad. When you reboot you should be out of Guided Access.
It seems that there's a glitch if you have triple-click assigned to any other functions in the Accessibility options (VoiceOver, Invert Colors, Zoom, or AssistiveTouch). If you have one of these turned on you cannot exit guided access with the triple click. To solve the problem, go into Accessibility options and turn off (uncheck) the other options for Triple-Click.

Hi, am having the same problem with ios7 ipad 4. Am stuck in an app with guided access, triple clicking just brings up a message in the middle of the screen with 'Accessibility options' voice over, guided access, cancel' on it. This doesnt do anything whatever is selected from those options.
I have tried the advice of DMcVicker above but when the ipad restarts it slides the screen across itself and goes back into the app I'm stuck on so I cant get into the settings to change anything...HELP!!!
