The fact that the iPad was ICE cold may have something to do with orientation issue. You might have waited until the iPad was a little closer to room temperature before you tried setting it up.
No - what you are experiencing is not a common problem. I would try rebooting the iPad (hold down on the sleep and home buttons for about 10 seconds until the Apple logo appears on the screen) and if that fixes it, I wouldn't stress about it. However, of the problem does recur with any frequency at all, contact Apple or take it to an Apple store to be looked at and evaluated.
