The fact that the iPad was ICE cold may have something to do with orientation issue. You might have waited until the iPad was a little closer to room temperature before you tried setting it up.
No - what you are experiencing is not a common problem. I would try rebooting the iPad (hold down on the sleep and home buttons for about 10 seconds until the Apple logo appears on the screen) and if that fixes it, I wouldn't stress about it. However, of the problem does recur with any frequency at all, contact Apple or take it to an Apple store to be looked at and evaluated.

Well, it has warmed up now and I have rebooted the iPad. I'm shocked that it still won't change orientation. I've had this problem off and on with my iPad 2 that's over 2 years old now but even that goes away when I reboot it.
I guess I'm going to have to call Apple...
