Delete app from Cloud or iTunes Match?

I have an app I've deleted from both my iPhone 4 (iOS 6.0.1) and off iTunes (11.0.0.163) on my PC (Windows 7 Pro 64 bit) but it continues to show up on my iPhone under "Apps > Purchased Apps > Not on this phone"  beside a Cloud icon with a download arrow on it.  I do have iTunes Match, and it is turned on.

 

The app is not on my phone nor does it show up in my apps library in iTunes.  If I click the Cloud icon next to the app on the "Apps > Purchased Apps > Not on this phone" it will download and install on my phone and in iTunes.

 

I have tried deleting it from both my phone and itunes, then backing up itunes both to my PC and the Cloud and syncing without the app present.  I continues to stay in the "Apps > Purchased Apps > Not on this phone". 

 

I have also tried a hard reset. No luck.

 

Anyone know how to delete it?

    You can't delete an app from your purchase history, you can only hide it from view using itunes (see http://support.apple.com/kb/HT4919).

    Right, that's why I mentioned to do it in iTunes rather than on your phone.  Since iOS 6 you can no longer hide purchases on your phone.  I assume that after you hide it in iTunes is will no longer appear on the purchased list on your phone either.
