In reading an article on CNN earlier this evening, T-Mobile in the US is supposed to start selling and supporting the iPhone after the first of the year. It may be that they are testing things and this is causing the intermitent on and off you are experiencing. If a reset isn't working to clear it, I would give T-Mobile a call and see if they can explain it. VVM is a carrier supported function, so they have to be doing something to trigger it.

We are using two factory unlocked iPhones (5 and 4s) on TMO and have experienced the same behavior over the past week or so. Our geographic area was recently refarmed by the way (most of it anyway...its ongoing).
I called TMO customer care and was basically told the same thing as in ChrisJ4203's reply. I was told to expect full functionality but not given a clear timeline.
While looking at the VV screen on the iPhone, a message box appeared briefly stating that visual voicemail is not currently available (i.e., the service is down). I suspect that is from TMO as they implement the service. Previously, tapping the voicemail button in the phone app did not display the VV menu; it only called TMO voicemail.
The latent voicemail badge are a problem for us as well. It does eventually appear and, when it does, tapping the voicemail button dials TMO directly as you stated above. Of course you can dial 123 (or just hold the "1" button) at any time to check voicemail as a workaround for sorts.
Do you have VV service now?

My wife says it's been up and down since my original post, but it must be changing less frequently because she complains about it less. (She also goes days without getting voicemail, so it's likely there have been times she didn't even notice it.)
She did tell me the other day she's been seeing 3G show up more often, and I know TMO is refarming our area a lot right now. I just wish they could figure this out.

Thanks for the update.
I'll try to post if we see consistent VVM service...actually, any VVM service since we have only seen the screen; not a populated list. TMO is supposed to officially offer the iPhone in April based on statement back in January, so I assume they will want VVM (the only missing feature) up and running.
