All this depends on how you create your feed. If you are hand-writing the feed then you can edit it in any plain text editor. If you are using a podcast creation program or service you will have to work within its paramaters, and it may not let you do what you suggest.
You can change the titles of your current episodes easily enough (don't change the 'guid' tag or people who have already downloaded them will find themselves downloading them again - if you don't have 'guid' tags this will happen anyway).
If you want to add older episodes and get them in order you will need to make sure that the 'PubDate' tag contains the date the episode was originally updated, not the date it's now added to the feed.
I don't know anything about FeedForAll, I'm afraid.

Also, if i want to delete any episodes, do I just pull them off of my server and remove the entires from the RSS feed?

I used Podcast Maker, but it hasn't been updated for some time and I believe it may be flaky with Mountain Lion (I stopped updating my podcast over three years ago). I don't really have experience of anything else. Most of these programs and services don't allow you direct access to the feed, though if everything is working properly you don't need it. There are online services, and actual programs like Podcast Maker - you can make podcasts in iWeb or RapidWeaver, for example, though I don't think either is as good. A lot of people use WordPress, but I see a lot of confused results from it: it can be made to work but by the number of people who've run into difficulties it's evident that it's not easy.
To delete episodes, remove them from the feed. With that done it doesn't matter whether the media files are still there or not, as there is no reference to them, but obviously you may as well remove them from the server.
When you delete episodes they will be removed from immediately for new subscribers but the Store will take a couple of days to catch up. People who have already downloaded the episodes you've removed won't lose them, of course.
