The same thing happened to me... I have a last played date and time, but my play counts have been erased. I've been meticulously keeping track of my play counts for the past five years and I am very unhappy about this (to say the least). If anyone has any idea how I can restore my playcounts, I would greatly appreciate it.

Same happened to me but with roughly half of my songs. It's particularly irritating because I had a ridiculous amount of plays on certain songs and I use play count to make playlists. Very very unhappy about this. Hope there's some way to get them back and Apple sort this issue out soon.

It's a Cloud issue in iTunes 11. To prevent it from happening again, go to iTunes > Preferences > Store and uncheck the option to Show iTunes in the Cloud Purchases.
For any tracks that have already been affected, there's a script for the Mac that you can use to set the play counts for tracks. Just do a Google search for applescripts play counts.

This can't be the whole issue. I have never had that setting checked in my preferences and my play counts are still being erased. As others have mentioned, I have been keeping track of play counts, organizing by play counts, and created playlists by play count for years. This is very frustrating.

Check View > Hide Items in the Cloud as well.
It's odd that it's still happening to you. I unchecked that preference (which is on by default) and haven't had the issue recur since. Before that, I was suffering almost daily play count resets.

Yeah, that is odd. None of my cloud preferences are checked. I don't have any of the cloud-related "View Options" checked and I don't have the show purchases in iCloud option checked in preferences. And it's not like I've turned those off recently; I have simply never had them on. I just don't understand it. I'm worried that Apple will eventually fix the bug but that the fix won't restore already lost play counts, which is a huge bummer to me. I'll just have to wait and see I guess, unless anyone has any other suggestions.

The only other thing that I can think of is that the iCloud view options were checked when I first downloaded iTunes 11 and that I turned them off right after. The play counts may have been lost in the interim without me noticing until now. I just don't ever remember going into "View Options" and changed that.

That would be my guess. You might have already thought of this, but I'll throw it out there anyway.
When this was happening to me daily (before I unchecked that preference), I made a Smart Playlist like this:
Plays is 0
Last Played is After 12/01/2004 (around the time I first installed iTunes)
That gave me a snapshot of what songs had been affected that day. I then tediously reset the playcounts (using one of Doug's scripts) to what I guessed was close to accurate.
I still have that playlist and check it often. It's been empty since I unchecked that preference.
