    Yes, the internal drive is dead or dying. You need to replace it.

    It does appear that a new HDD is in your future.  You may take it to a repair facility for verification and/or replacement.  If you are handy, the switch can be done by yourself and at less cost.  Check OWC.com, Newegg and Amazon for replacements.  You will need #00 Phillips and a #6 Torx drivers for the installation.

    Yes, you do have to format a new HDD.  Open Disk Utility>Erase and format it Mac OS Extended (Journaled) and the proceed to install the OS.
