The 40Mbps speed is most probably due to WiFi interference, other users on the network etc. No ISP ever delivers it's stated maximum speed e.g. I have 100Mbps Internet but only ever get around 75Mbps.
Try checking with iStumbler for WiFi interference.
With regards to your other question, I need to confirm a few details. Just to make sure I've got it right:
AirPort Extreme --> AirPort Express --> 2nd AirPort Extreme?
And you want it to be AirPort Extreme --> 2nd AirPort Extreme and AirPort Express?
Are you using Ethernet or WiFi to extend?

as for chain of connection
it's all wireless
Airport extreme to airport express (old model) to airport express (brand new one)
the Airport extreme 2nd generation is totally on another floor and speed is undestandably slower

Got IStumbler but not too sure what I'm supposed to see from it
my network is listed 3 times (including the guest network)
one shows 57%, the other one 73%
guest shows 59%
And i'm sitting right next to my modem
2 more listing for my network

iStumbler can check for WiFi interference. Check your networks are on channels that no other network is using. The 2 networks are the 2.4GHz band and the 5GHz band, you should consider renaming the 5GHz band to another name and using this if possible.

I'm afraid this is starting to sound chinese for me
and I did not study that language very much
I'm out of town till tomorrow and will do some research to understand this
especially how to changing the channel and the band
