Have you tried disconnecting the USB headphones? Could be they are controlling the system for sound output to the USB port.
My current MBP and iPod will be the last Apple devices I ever buy.
Good Luck and Best Wishes.

Reset SMC. http://support.apple.com/kb/HT3964
Choose the method for:
"Resetting SMC on portables with a battery you should not remove on your own".
Applications/Utilities/ Audio MIDI Setup.app
Audio Devices window
Click the Built-in Output.
Under Mute any box checked? If so, uncheck it.

I just had this happen out of the blue on my one month old 13" MBPRetina. I researched everything on here and the internet and then it occurred to me to try an old PC trick, which is restart the computer (I usually put it to sleep) to reload drivers and it fixed it for me...don't know if this will help anyone else but good luck.

Manually deleted old files likes ones below from
Library > Preferences > Audio (these were outdated - spread the word!)
Library > QuickTIme
FCP Uncompressed 422.component
Google Camera Adapter 0.component
Google Camera Adapter 1.component

na, am not goin to delete files i dont know.
At the moment i only get sound via usb headphones and no solution worked so far
After this i guess i ll dispose of my macbook and buy a fairer priced but working and repairable windows laptop.
I just cant believe how ignorrant apple is. This and the dvd incident were not just happening to very singular users but i found plenty of those in boards, none which received any help from apple if not under care plan and the dvd swapped.
They already have the unique benefit of a hardware environment which they decide on, they design. Not releasing patches and updates which work properly but render sound or dvd drives useless is just not very impressive nor giving any value proposition in the price they charge.
Utterly disapointed by this 2100Euro machine
