Had to contact Bose to replace the headset under warranty because Bose & Apple could not determine what the problem was.
The replacement unit has worked flawlessly, but in the past week I am seeing signs of the same problem starting up again and will have to monitor it carefully and see if Bose has another firmware update available that may resolve this issue.

Thanks. I have iOS 6.1 and I had same issue but now it is working perfectly after upgrade the Bose Headset

Try holding the talk button on the Bose for 5 seconds while in pairing mode especially on IOS 6.1 on later devices. iPad 2 and iPhone 5 worked for me. You couldnt even see the Bose Bt2 until then.
