Sound out of sync in timeline

Whenever I place a clip in the timeline, regardless of method, the sound gets out of sync. It's in-sync in the viewer, but not in the timeline.

 

This started as an occasional problem. Now it happens 100% of the time. My work has come to a screetching halt.

 

Any suggestions out there?

iMac (27-inch Mid 2010), OS X Mountain Lion (10.8.2)

    Verfiy the following:

     

    A) The audio and video codecs work natively in FCE.

     

    B) The frame rate of the video matches the Sequence settings.

    Since neither FCE or FCP don't work with MP4 files natively, using one in a Sequence will cause the need to render often.  Best to use Compressor (or other app) to convert the video to the proper format AND frame rate.
