There is no fee/cost for activating an account, it's free - and there is no cost for using an account, apart from for any non-free items (music, apps etc) that you decide to buy from the store. If you only download free items and don't purchase anythin within them (some apps allow in-app purchases and/or subscriptions) then you shouldn't receive any charges. When you create an account you should receive an email to that email address which should have a 'verify now' link in it.
When you add or change the credit card details on your iTunes account you may receive a small temporary store holding charge to check that the card details are valid and that the card is active, but that should disappear within a few days : store holding charge.
