That is actually two different panic reports slammed together. Both are inter-process co-operation problems, but they occurred in slightly different areas and are slightly different symptoms.

This could be a graphics Hardware problem, or possibly a RAM memory problem, or maybe something else entirely.

You should try to run Apple Hardware Test (long version), although false negatives are common. You should try running all-night memory tests.

You should consider finding time to make an appointment at the genius bar for further diagnostic testing.

Thanks very much Grant, when this happened, I recall the computer heating up substantially. I wonder whether the 16gb of RAM that was installed when I ordered it has anything to do with it. (The default option was 8gb.) I have tried turning off the screensaver, etc, but at the moment Calendar, Contacts, Chrome and Mail are all not responding. Trouble is I really need this for the next few days. I have so much work on. I hope it holds out till Wednesday.

This has been hapenning to me which is how I found your post. To me it feels like it's graphics related. I've disabled automated switching and will see if that helps...

I'm getting the same issue and I'm not using Chrome ... mine happens whenever the laptop wakes up from sleep mode ... Only thing I can think to fix it is to change the energy saver settings to never let it sleep ... which is compelte BS, Apple has an MCS update which I just applied today and it did not fix the issue.

This is happening on my work laptop.  I'll have to ask our IT dept to send it back under warranty and give me a windows laptop which ****** ME OFF to no end.

No need to de-install any software etc, same problem suddenly popped up an a 3 month old MBPro Retina 15"; even with a freshly installed 10.8.3:

-one day system started to crash with kernel panics again and again

-hardware test at the so called "Genius" bar found no error

-as mentioned even a completely newly installed plain 10.8.3 did not solve the problem:

sudden crashes while working or even doing nothing, and nearly after every wake-up either

    •      "AGC GPU REGISTER RESTORE FAILED" or
    •      "NVRM[0/1:0:0]: Read Error"

    My solution: new hardware.

Apple replaced the complete logic board, now two days without that problem (only now the mouse-pointer disappears whenever I switch to iTunes, but this old bug is discussed in another thread. How awful!)

    Specs:

    MbPro Retina 15", NVIDIA GeForce GT 650M + Intel HD Graphics 4000, i7 2.3GHz, 8GB RAM

I was one of the many people who had a crashing MBPr. It was a problem isolated to OSX 10.7 Lion. I have not had this issue with 10.8 Mountain Lion, nor has my friend who also owns a MBPr (and who had the issue in 10.7). So if anyone has a crashing problem, and they've installed a fresh, unmodified copy of Mountain Lion 10.8.4 (at the time of writing this), it's hardware related.

    Hi Brandon:

I had the same issue with my flash-based MacBook Pro at work, also with retiina display ... seems like part of the energy saving feature is to switch between a high performaqnce graphics card and a more moderate Intel graphics card when needed, this switching video display drivers, especially when the system is waking up from sleep or screen saver, causes a bad crash .... I eliminated all screen savers and Energy Saving settings liek putting the computer or display to sleep, and I ahve never had the problem since ... ***** since it screws your batery life ... but at least the flash based systems have much longer battery life anyway ....  Just try turning off all the sleep mode energay stuff and that should fix it ....  good luck

Thank you thewesternfront, I'll keep your suggestion in mind.

However, I don't think my issue is caused by sleep issues etc as I've tried sleeping my laptop and waking it up a few times, no crashes yet.

I've reinstalled the OS and it seems that the problem is solved. Will report back if I encounter any crashes.

Okay...I guess I have the same problem as you.

It restarts itself on sleep mode (auto-sleep mode) however it doesn't restart when I sleep it manually (click on apple on top left and sleep).

Is this the same for you?

    BrendonSG-

you have a very unusual panic reason:

    panic(cpu 0 caller 0xffffff802a6a2a6a): "pmap_pv_remove(0xffffff803ea82000, 0x116551000, 0xc28c, 0x800000000c28c024, 0xffffff811c3b3da4, 0xfffffe9e72101a88): pv not on hash, head:

Ps and Vs are classic synchronization data structures. In this case, it appears that routine went to remove one and it was not there. This is bizarre.

In addition, the routine where this occurred is:

BSD process name corresponding to current thread: ReportCrash

So it crashed in the middle of reporting a crash.

In my opinion, there is something wrong with your computer. I suspect you have a RAM memory problem. This theory would be bolstered by seeing different panics in different locations for different reasons.

The panic logs are stored, and can be accessed using the procedure described in this article. I suggest you read out all the panics in your logs and compare them. If the same, this may be a problem that a re-Install would solve. If different, that suggests a memory problem.

    Hi Brandon:

Yep that is what was happening to me as well,  and to all of my coworkers who are using the retina, flash-based MacBooks  I was SO glad that this was my work laptop and not a personal machine becuase if I had spent the kind of money they did on my own machine and it crashed several times a week, I would be LIVID, and sounds like you ahve had to put a lot of personal effort into diagnosing your issues as well, sorry to hear that :-(

We passed the word around our office to shut off energy comp[liance settings and everyone's issues were resolved ... really just boils down to bugs in the graphics driver waking up properly I believe ...

Not sure if this issue is the cause or matches the signature of your posted crash log but hopefully it will help with any crash on wake problrms you will encounter  :-)

    thewesternfront-

kernel panic is not ONE problem -- it is hundreds to thousands of different probelms.

Do you have the same obscure panic reason, and panic in the same routine?

Please post a panic log, see the link above for how to retrieve the latest one.
