From whom did you purchase "insurance"?

    Ask them.

Ask T-Mobile..   Generally the insurance only covers "manufacturing" defects and if it came "automatically" then the insurance probably isn't worth much.

You will need to go to a T-Mobile shop to handle your insurance claim.

Assuming it is T-Mobile in the US, they will not be officially selling the iPhone until April 18th. So it was probably an independent T-Mobile reseller. Thus, you will have to ask them what their policy is. Apple offers AppleCare+ which provides a 2 year warranty on the phone plus a damage replacement plan where they will give you a replacement phone for $49.

Depends upon the type of insurance that he purchased from T-Mobile and the required deductible. Only they would know.

That's entirely up to the store where it was purchased, as it is their insurance. If you take it to Apple they will replace it for $229 (assuming that the insurance was not AppleCare+, which costs $99).

I didn't ask, but I don't believe this iPhone was purchased in the US. T-Mobile is an authorized carrier in several countries in the UK and AppleCare + is not available there. The deductible here is $49.
