Prolonged use of the phone for processor heavy tasks will cause the phone to heat, however it should not get too hot to touch. This heating will also occur when charging.

Contact Apple Care and request a diagnostic report, they should be able to see if your phone is running at unsual temps and properly identify the source.

You should try backing up the data, restoring the device as new and try if that solves the problem. NB: set up as NEW device, don't restore with backup, because a software issue will also be in the backup. If the issue persists after restoring as new, the device needs service.
