Try doing a hard reset:
Press and hold both the Home button and the Sleep/wake button simultaneously for about 15 seconds and release when the Apple logo appears.

You are sure you have set the Settings > General > Use Side Switch to: Lock Rotation and then switch the direction to make see that it is unlocked? Or just make sure it says Mute.
Is there a lock symbol in the upper right portion of the screen? If so, the rotation has been locked.

In my case on an iPad 2 which has stopped rotating this doesn't work nor doe a hard reset.
Can it be a hardware problem?

If all of the settings are correct and it will not rotate for any app, then it may be a problem with he position sensor. That needs to be diagnosed by a tech at an Apple store genius bar.

Go to the ExpressLane and file a service request, https://expresslane.apple.com so you have it on record Nd also se what response they give.
