Mac appears to be sending spam but none in sent folder but are from his IP

I have a friend on a Mac, using Outlook and IMAP whose email contacts are getting spam from him. There are none in his accounts sent folder but an examination of the full header of one of these sent spam emails shows it is coming from his mail host. (see below) My question is what logs do I need to look at now that I have a time and date.? Any other hints?

My guess is one of three things, someone is logging in remotely and sending and deleting sent
