The necessary links/menus you need to navigate to after clicking the Report a Problem link can be a little tricky to find. See

I had previously logged into iTunes etc., then clicked the small 'report a problem' link for the purchase in question, same as the instructions in your Refund... link above. BUT, the link instead went to the iTunes Support 'Welcome' page, not a form relating specifically to the problematic App purchase.
I searched for ages in iTunes support without success (hence my initial frustration) and concluded with the post above.
Thanks for your reply. I have emailed iTunes Support and am awaiting their reply.

Or you can try contacting iTunes Support via this page : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then Purchases, Billing & Redemption
