Could you please upload your AppleScript to a few different mirrors (I recommend Mediafire, Dropbox, zippyshare, depositfiles.com, and ge.tt)?

4shared gives this lovely note: "'LazyApple.app' is unavailable. This file is no longer available because it's identical to file banned because of claim."

    Thanks so much in advance!

    Sure, here's the link:

    https://dl.dropbox.com/u/17687460/LazyApple/Sound%20Balance%20Reset.zip

If you add this to your Login Items under System Preferences > Users and Groups, it will fix the bug from affecting you on restart.  It doesn't fix it from happening when your laptop is asleep and you plug it in to the display, though.  I looked into how to make that happen - it takes terminal commands and installing a seperate program (also through terminal).  And it makes your System Preferences windows pop up every time your computer is opened.  Annoying.  I'm sure someone with actual programming knowledge could do better though.

Everyone that sees this, please go here: http://www.apple.com/feedback/macosx.html, and report this thing under 'Bug Report'.

This makes a strong case for software being the culprit.  A couple weeks ago I plugged my computer into the cinema display, and it worked the way it should have.  But every day since it's the same bug.

So, go here: http://www.apple.com/feedback/macosx.html, and report this thing under 'Bug Report'.  Then report it again and again, using other peoples names and credentials.  Thankyou.

More details on what I've seen:

2011 MacBook Air, plug into Cinema display while laptop is closed/asleep - sound balance shifted right

2011 MacBook Air, plug into Cinema display while laptop is already open/on - sound balance fine

2011 MacBook Air, plug into Cinema display while laptop is already open/on, then close lid (switches to just external monitor) - sound balance switches to right channel only

I also use the monitor with a windows box too:

Windows 7 box, plugging into ATI Radeon HD 7970 - sound balance always fine

I have the issue too, sound switches to full right channel for no apparent reason. It does not appear to involve attaching or removing anything from the computer. I have SoundSticks connected by USB, headphones in the sound jack, a late 2012 27" iMac 3.4 GHz i7 16GB RAM 1.12 TB Fusion Drive with a late 2005 20" Apple Cinema Display always attached through an Apple Thunderbolt to DVI connector. I never remove any of these items from the iMac. The sounds I play on the computer are mostly iTunes music or videos, YouTube or news on websites, and occasionally EyeTV. I haven't noticed any connection to starting or stopping any sound source and the balance changing itself.

    In Singapore

I have a late 2012 iMac 27", and I am also facing the same issue. Sound automatically shifted to right speaker occasionally.

I am using iMac built-in speakers.

Hope there is a permanent solution soon.

For my 2012 rMBP, OS X 10.8.4, I have dual displayport LED Cinema monitors at work and the audio shifts to the right channel only when I restart my computer. Once I center the balance in System Prefs, it will stay "fixed" until I reboot. Stays "fixed" through sleep, undocking, etc. If I reboot, the audio goes back to right-channel.

I also have a Thunderbolt display at home and the audio is always correctly centered. My Thunderbolt display is almost always connected to the port on my rMBP closest to the power connector.

I just trashed the com.apple.audio.AudioMIDISetup.plist and Audio Folder in /Library/Preferences/Audio and I've rebooted twice and it seems to fix it!

I have the same problem, the sound balance will shift totally to the right.

I do not have any type of Apple Display.

I have an early 2011 MBPro 15". I have not connected any external displays or audio devices in a LONG time. I have however routed the audio to other devices using bluetooth.

All system software is fully updated. I also just reset the SMC and PRAM to see if that makes a difference.

This is technically "Solved", for me.    But it's not the solution you want to hear:

I'm guess most everyone that's been reformatting their systems on this forum has been restoring via a Time Machine backup during the new install process (because, why wouldn't you - it's freggin annoying to get all your settings back).

Problem is I'm betting that restores the bug as well.  I was having more problems with the OS unrelated to the sound balance, so I started from scratch.  I didn't restore any of my special system addons, SSD hacks (TRIM), etc.  I didn't even install a font manager, since it hijacks the system one.

And yeah, the bug is gone ;/

I'll be slowly re-installing some of my previous software.  If any of it starts up again, I'll let you know.

My guess is something happened in an OS update with certain system software configs.  Not saying this shouldn't be figured out and patched, though!
