Well, when I first got my iPhone 5, one of the first things I did was to enable the 4 digit passcode. I entered the code twice and it gladly accepted it, and I was happy. About half an hour later, I pulled out my iPhone to do something with it, and suddenly it wouldn't accept my 4 digit code. After several incorrect attempts, and some waiting (I was up to the 15 minute wait between attempts) I was able to figure out where my fingers hit when trying to enter my passcode, and got my iPhone unlocked. I then promptly changed teh passcode to wheat it should have been set to the first time, and all was happy. For me the iPhone 5 screen digitizer was taking my finger hits as lower on the screen than I thought I was tapping, so my code had replaced a '1' with a '4'.
