Close All Open Apps... Perform a Reset... Try again...
Reset ( No Data will be Lost )
Press and hold the Sleep/Wake button and the Home button at the same time for at least ten seconds, until the Apple logo appears. Release the Buttons.

Tried toggling mute buttons, clean dock connector and mic near headphones connector, plug/unplug headset but no luck. I did hard reset, restore, restore and another series of hard resets but still no luck.

Then it looks like a Visit to an Apple Store or AASP (Authorized Apple Service Provider) is required...
Be sure to make an appointment first...
